TRADITIONAL CHINESE MEDICINE & PERIMENOPAUSE
A gentle guide to understanding your body's signals during perimenopause through the lens of TCM

Perimenopause doesn’t look the same for everyone. One woman feels like she’s burning from the inside. Another feels exhausted before the day even begins. Another feels heavy, stuck, or emotionally raw.
In Traditional Chinese Medicine, this isn’t random. Your unique pattern of symptoms reflects your body constitution — a deeply personal map of how your vital energy flows, where it gets stuck, and what your body most needs to come back into balance.
There are nine constitutions. As you read through them, notice which one — or which combination — feels most like you. This isn’t a diagnosis. It’s an invitation to listen more closely to your body.
THE NINE CONSTITUTIONS
🌿 1. Qi Deficiency — The Energy Low Type
Not everyone going through perimenopause feels “wired and hot.” Some feel the opposite.
You may often feel:
• Tired, even after resting
• Short of breath or low stamina
• Spontaneous sweating
• Low motivation
You may also notice:
You may prefer warm foods, feel worse when overworked, and find that your energy simply doesn’t replenish the way it used to. This isn’t laziness. This is your body asking for more energy support.
· · ·
🔥 2. Yang Deficiency — The Cold Type
Not everyone going through perimenopause feels hot. Some feel cold. All the time.
You may often feel:
• Cold hands and feet
• Low energy, especially in the morning
• Sluggish metabolism or weight gain
• Needing warmth — in food, environment, even people
You may also notice:
You prefer hot drinks over cold, feel worse in cold weather, and feel noticeably better with rest and warmth. This isn’t just getting older. This is your body asking for warmth and support.
· · ·
🌙 3. Yin Deficiency — The Restless Heat Type
Restless, warm, and unable to settle — especially at night.
You may often feel:
• Night sweats
• Hot flashes, especially in the evening
• Dryness — skin, mouth, eyes
• Light or interrupted sleep
• Mind feels “wired” even when the body is tired
You may also notice:
You feel worse at night, crave cooling foods, and your mind stays busy long after your body wants to rest. This isn’t just hormones. This is your body asking for cooling and nourishment.
· · ·
🌧️ 4. Phlegm-Dampness — The Heavy Type
Some changes feel heavy — not just physically, but in energy too.
You may often feel:
• Heaviness in the body
• Weight gain that feels hard to shift
• Bloating or sluggish digestion
• Low motivation
• Energy that feels “stuck”
You may also notice:
You feel worse after certain foods, crave sweets or comfort, and your energy feels stuck rather than flowing. This isn’t lack of discipline. This is your body asking for movement and lightness.
· · ·
⚡ 5. Damp-Heat — The Inflamed Type
Sometimes it’s not just heat — it’s heat with irritation.
You may often feel:
• Internal heat or inflammation
• Irritability or short temper
• Breakouts or skin issues
• A feeling of heaviness with heat
You may also notice:
You don’t tolerate greasy or rich foods well, feel worse in humid weather, and your body feels generally unsettled. This isn’t random. This is your body asking for balance and clearing.
· · ·
🌊 6. Blood Stasis — The Stagnation Type
Some symptoms don’t move. They linger.
You may often feel:
• Sharp or fixed pain
• Tension that doesn’t release easily
• Darker complexion or under-eye circles
• A sense of stagnation in the body
You may also notice:
Symptoms come back in the same place, discomfort feels “stuck,” and there may be an emotional heaviness or sense of holding on. This isn’t just aging. This is your body asking for circulation and flow.
· · ·
🍃 7. Qi Stagnation — The Emotional Type
Sometimes it’s not physical first. It’s emotional.
You may often feel:
• Mood swings
• Irritability or frustration
• Tightness in chest or shoulders
• Feeling emotionally “stuck”
You may also notice:
Stress affects your body quickly, your digestion shifts with your mood, and you feel noticeably better after movement or expression. This isn’t just stress. This is your body asking for flow and release.
· · ·
🦋 8. Special Constitution — The Sensitive Type
Some bodies are more sensitive than others. And that’s not a weakness.
You may often feel:
• Sensitive to foods, environment, or weather
• Prone to allergies or reactions
• Easily affected by changes
• More reactive than others
You may also notice:
Your body responds quickly — for better and for worse. You need more care with what you consume, and your system can feel delicate. This isn’t being difficult. This is your body asking for protection and awareness.
· · ·
☯️ 9. Balanced Constitution — The Harmonious Type
This is what we’re often aiming for. But it’s not about perfection.
You may often feel:
• Steady energy
• Good digestion
• Emotional stability
• Restful sleep
• Quick recovery
You may also notice:
Your body adapts well, you recover from disruption with relative ease, and you generally feel in flow. But balance isn’t fixed — it shifts with life, stress, and time. Perimenopause can change that balance. And that’s okay. Because awareness helps you come back.
———
Reading through these, you may have recognised yourself in one constellation of symptoms — or in two or three, overlapping and shifting depending on the season of life you’re moving through.
That’s exactly how TCM sees it. We are not static. Our constitutions reflect our history, our habits, our environment, and the demands placed on our bodies over decades.
Perimenopause is not a malfunction. It is a transition — and like all transitions, it asks something of us. It asks us to pay attention. To adjust. To offer our bodies a different kind of care than the one that carried us this far.
The question isn’t what is wrong with me. It’s what is my body asking for?
That’s the beginning of everything.
